Licensed Practical Nurse (LPN) – Home Care

About the Role

Elderly Savior Homecare is seeking compassionate, skilled, and reliable Licensed Practical Nurses (LPNs) to join our growing home care team.

In this role, you will provide hands-on patient care, support care plan execution, and play a critical role in monitoring patient health and progress in a home setting. This position is ideal for an LPN who is detail-oriented, patient-focused, and comfortable working independently.

Key Responsibilities

• Administer medications and monitor for side effects or adverse reactions
• Monitor and record vital signs and patient condition
• Observe and report changes in patient health, behavior, or progress
• Assist in implementing and following individualized care plans
• Collaborate with RNs and other healthcare professionals to support patient care 
• Perform wound care , including cleaning and dressing wounds as needed 
• Maintain accurate, timely patient documentation and reports
• Educate patients and families on care routines and treatment adherence
• Ensure a safe and clean care environment 

Qualifications

• Active Licensed Practical Nurse (LPN) license – State of New Jersey
• Graduate of an accredited Practical Nursing program (ADN or higher preferred)
• NCLEX-PN passed
• Minimum 1 year of nursing experience (home care preferred) 
• Strong understanding of patient care, medication administration, and documentation
• Ability to work independently with minimal supervision
• Excellent communication and interpersonal skills
