Job Description

GENERAL FUNCTION:

The Licensed Practical Nurse provides nursing care under the supervision of an RN or physician, delivering treatments and monitoring patient health in the home setting.

DUTIES AND RESPONSIBILITIES:

  • Provides skilled nursing care such as wound care, injections, catheter care, and medication administration.
  • Monitors patient conditions and reports significant changes to the supervising RN or physician.
  • Documents nursing care and interventions accurately and in a timely manner.
  • Assists in implementing the patient’s care plan as directed by the RN.
  • Educates patients and families about medications and care techniques within scope of practice.
  • Maintains infection control standards and patient safety during all care activities.
  • Participates in training and performance evaluations as required.

This is for a client on Evansville Northeast side off of Lynch Road assisting 3x daily- everyday through April 25, 2026. Must be comfortable with IV Infusion and Would Care. We're looking for 2-3 individuals to fill this position. 

KNOWLEDGE AND REQUIRED TRAINING:

Must be a licensed Practical Nurse (LPN) in the state of Indiana. Requires understanding of nursing procedures, home health protocols, and documentation standards. Must be CPR certified.

PERSONAL RELATIONSHIPS:

Works under the supervision of an RN. Maintains communication with patients, families, and interdisciplinary team members.

PHYSICAL DEMANDS:

Requires the ability to perform patient care activities such as lifting, turning, and transferring patients. Must be able to drive to patient homes.

WORK ENVIRONMENT:

Work is primarily in patient homes. May encounter varying home conditions and potential exposure to infectious diseases.
